This template can help your partnership assess its current status with regard to sustainability of its regional or local initiative.  When your planning team uses this tool along with other available resources and information, the result should be a shared understanding of the partnership’s current situation that prepares the team for strategic planning.

The Sustainability Self-Assessment is organized as follows: (1) stakeholder involvement, (2) leadership for sustainability, (3) implementation of STW components across the system/community, and (4) integration and realignment of resources to support the system.
