This survey of team member perceptions helps a STW leadership team assess its effectiveness as a team. 

The survey is appropriate for leadership teams or governing boards encompassing a broad coalition of business and education partners who oversee or advise a STW initiative. It provides feedback on Team Structure, Mission, Goal Setting, Empowerment and Communication, Logistics, and Team Member Expectations.
