Hardware Contract

I, (name) _____________________________, with student ID number: ______________________, understand that this course requires the use of hardware and software. I have signed this sheet below as my understanding of the obligations and conditions under which I will have access to this material.

HARDWARE. I will be provided with University-owned Palm Pilots and Palm keyboards. This equipment is available to check out for the duration of this semester only. I will return the hardware in perfect working order to Prof. Szuchman (or his designee) at the end of the semester, no later than 5:00 pm on the date the final papers are due. Failure to return the equipment in perfect working order will result in the placement of a “hold” which will prevent me from registering or using University facilities. My name will be also turned in to accounts receivables for collections.
