Synergy Patterns
Upper Extremity
Flexion Pattern:
- elbow flexion
** strongest (brachioradialis and biceps)
- forearm supination
- glenohumeral abduction
*weak*
- glenohumeral external rotation
*weak*
- scapular retraction
and/or elevation
- usually wrist and finger flexion
Extension Pattern:
- elbow extension weak (triceps)
- forearm pronation
- glenohumeral horizontal adduction
(pectoralis major) *strong*
- glenohumeral internal rotation
(pectoralis major) *strong*
- scapular protraction
- usually wrist extension and finger flexion
Factors that Influence Recovery After a Stroke
Positive Outcome more likely if there is:
- early return of muscle tone (within 2 weeks)
- early return of muscle function (within 2 weeks)
- intact sensation
- intact perception
- minimal spasticity
- intact cognition
- intact body scheme
- some spontaneous use of affected UE
- some selective motion in affected UE
Negative outcome more likely if there is:
- prolonged muscle tone problems
- apraxia (difficulty with motor planning)
- poor sensation
- receptive aphasia
- unilateral neglect
- poor body scheme
- poor spatial relations
- poor selective motor return
- continued incontinence

Ability to monitor behavior
Ruler of social acceptability
(where and when things are appropriate)
Judgement
Motivation
Planning
Ability to care about things
Abstraction
Left Brain Functions:
Language
Analytic processes
Rational thinking
Classification processes
Controls right body side
Feels right body side
Reading
Grammar
Praxis (motor planning)
"splitter" breaks things into parts
Right Brain Functions:
Controls and feels left body side
Visual perceptual functioning
