Upper Extremity

Flexion Pattern:

  1. elbow flexion** strongest (brachioradialis and biceps)
  2. forearm supination
  3. glenohumeral abduction *weak*
  4. glenohumeral external rotation *weak*
  5. scapular retraction and/or elevation
  6. usually wrist and finger flexion

Extension Pattern:

  1. elbow extension weak (triceps)
  2. forearm pronation
  3. glenohumeral horizontal adduction (pectoralis major) *strong*
  4. glenohumeral internal rotation (pectoralis major) *strong*
  5. scapular protraction
  6. usually wrist extension and finger flexion

 

Factors that Influence Recovery After a Stroke

Positive Outcome more likely if there is:

  1. early return of muscle tone (within 2 weeks)
  2. early return of muscle function (within 2 weeks)
  3. intact sensation
  4. intact perception
  5. minimal spasticity
  6. intact cognition
  7. intact body scheme
  8. some spontaneous use of affected UE
  9. some selective motion in affected UE

Negative outcome more likely if there is:

  1. prolonged muscle tone problems
  2. apraxia (difficulty with motor planning)
  3. poor sensation
  4. receptive aphasia
  5. unilateral neglect
  6. poor body scheme
  7. poor spatial relations
  8. poor selective motor return
  9. continued incontinence
