Alphabetically indexed table, with 2 columns of cells. (Easily adapted to more columns &/or rows.)

- Has a "Top of Page" button at each indexed point, aligned to right side of indexed cell. (One version has this button aligned to right side of page with the indexed letter in the page center.)
- Has a "Go to Alpha List" button at each indexed point, aligned to left side of indexed cell.
