Request for Equipment Policies and Procedures

Reservations
  • Equipment request form must be submitted 24 hrs. in advance.
  • Equipment can only be used during office hours, Monday - Friday, 8:30 AM - 5:00 PM.
  • Users may be barred from using loaner equipment if they do not follow agreement form provisions or provide false information.
  • Faculty and staff may obtain loaner equipment from the University's A/V department in the Green Library.

Eligibility

  • Currently MBRS & MARC students can borrow equipment.
  • All users must complete and submit a request form.
  • All users must sign an equipment agreement form.
  • Users may be barred from using program equipment if they do not follow agreement form provisions or provide false information.
  • No checkout after 5:00 P.M. unless prior arrangements have been made.
  • Prior approval for overnight and/or weekend loaners is required.

Responsibilities

  • Keep the laptop away from water, fire, humidity, beverages, etc.
  • Do not drop equipment; keep it in a secured position.
  • If accidents occur, do not attempt to fix laptop. Bring it back to HLS II 395 and report the incident.
  • Equipment is due back no later than 5:00 P.M. on the check out date.
  • Due to high demand and high usage, return equipment on time.

Please note that this request form does not guarantee reservation of the equipment. After submitting the form, a confirmation email will be sent to you indicating whether the equipment is available for use.
