Please complete and submit nomination form. Incomplete forms will not allow the committee to adequately review your candidate's merits. Forms may be submitted by e-mail or by using the paper forms. Please see a committee member if paper forms are preferred. Current committee members are listed on the Library Student Ambassador Program.
Information within this form will be the only opportunity for you to advocate your candidate's chances. No interviews or requests for additional information will be undertaken. The Committee will not be contacting nominees or nominators.
Nominations will be accepted only during the Open Nomination Period of each semester.
