General Forms: Forms available through the Registrar's Office:
    Change of Major Form
    Change of Address form
    Change of Name form
    Change of ID form

Graduate Forms: Graduate students in Masters and Ph.D. programs must obtain a graduate packet from the Division of Graduate Studies located at UP campus in PC 236. This packet contains important forms that document the graduate students progress in their graduate program. These forms are used to ensure that the graduate student is on schedule, concerning the time frame that their program allots to obtain their degree. Ph.D. graduate students must fill out forms A through F, while Masters students must fill out forms C through F. The title of forms A through F are listed below:

Form A: Request to take Doctoral Candidacy Exam
Form B: Advancement to Doctoral Candidacy
Form C: Dissertation Committee Appointments
Form D: Request for Dissertation Defense
Form E: Result of Dissertation Defense
Form F: Dissertation Memorandum of Approval
