FIU
IMMUNIZATION POLICY
As a pre-requisite
to registration, Florida International
University requires all students
to comply with the following immunization
policy regulations from the Florida
Board of Governors regarding immunity
of:
Any written
document must be signed by a physician
(M.D. or D.O.),
a nurse practitioner or a registered
nurse and must be mailed, sent
via fax, or physically brought
to University Health Services on
either campus at least
four weeks . Phone calls
from your physician are not acceptable.

MEASLES,
MUMPS, RUBELLA:
All
students born after December 31,
1956 must
present documented proof of
immunity to Measles (Rubeola) and
German
Measles (Rubella), as described
below:
Acceptable
Proof Of Immunity consists of:
-
Proof
of
2 vaccinations (doses)
of MMR (Measles/Mumps/Rubella)
received
at least 28
days
apart
or 2 doses of Measles and
1 Rubella.
- Vaccinations must have
been received after your first
birthday
- Vaccinations
must have been received in
1969 or later.
- Proof of
immunity by way of a blood test
lab result (Measles and Rubella
Titer).
- A written
statementfrom a healthcare provider
documnenting a diagnosis of Measles
(Rubeola). This must include
date of diagnosis, be signed by
the
healthcare
provider and be on his/her
offical stationery. It is acceptable
for Measles only and does not
apply to Rubella.
Exemptions:
You will be exempt
from the pre-registration immunization
requirement for Measles, Mumps, and
Rubella, only if you meet any one of
the following criteria:
- You were
born before January 1, 1957.
- Medical Exemption:
You
must provide a letter from a healthcare
provider, signed on his/her stationery,
stating the medical reason(s) why
you are not able to receive the
Measles and/or Rubella vaccine(s)
and for how long - a permanent
or temporary medical condition
warranting exemption.
- Religious
Exemption:
You
must complete the Vaccine
Waiver for Religious Exemption
Form and provide a letter
from your church stating that you
are an active member of the church
in good standing and that
it is the tenet or practice of
that church that its members not
receive medical vaccinations. No
details of the religious tenet
or practice are required. The
letter must be written on the church’s
letterhead
and signed by the officiating clergy,
and it must include his/her address
and telephone number.
If
you are not a member of an
organized church or religious
sect, or if your personal
religious beliefs are not
shared by your church, you
may request an exemption
from fulfilling the required
vaccination(s) based upon
your personal religious beliefs
and practices. In this case,
you (or your parent or guardian
if under 18 years old) must
complete the Vaccine
Waiver for Religious Exemption
Form and provide a letter stating
that your personal religious
beliefs and practices do
not permit you to receive
medical vaccinations.
Temporary Deferments:
Temporary deferments
are acceptable for the following conditions:
- Documented
pregnancy or fertility treatment
- Documentation
of breastfeeding
- Documented
illness
Deferment status requests
must be submitted to the University
Health Services as least 4 weeks
prior to registration and the request
must be signed by a healthcare provider
and be on his/her stationery.
HEPATITIS
B:
All students
matriculated from July 1, 2008 or later,
must present documented proof of vaccination/immunity
to hepatitis B as described below:
Acceptable
Proof of Immunity consists of:
-
Proof
of a total of 3 doses of hepatitis
B vaccines
-
Proof
of immunity by way of a blood
test lab result
-
A
written statement from a healthcare
provider documenting a diagnosis
of hepatitis B. Must include
date of diagnosis, be signed
by the healthcare provider
and be on his/her official
stationery.
Exemptions:
Students declining to receive
vaccination for Hepatitis B must present
a signed Waiver
of Liability Form acknowledging
that they have received and read information
pertaining to the disease and despite
knowledge of the risks have decided
to waive receiving the vaccine (if
a minor, the waiver of liability must
be signed by the parent or guardian).
Temporary
Deferments:
Temporary deferments are acceptable for the following
conditions:
- Documented
pregnancy or fertility treatment
- Documentation
of breastfeeding
- Documented
illness

MENINGITIS:
All

Exemptions:
Students declining to receive
vaccination for meningitis and/or hepatitis
B must present a signed Waiver
of Liability acknowledging
that they have received and read information
pertaining to the disease and despite
knowledge of the risks have decided
to waive receiving the vaccine (if
a minor, the waiver of liability must
be signed by the parent or guardian).
Temporary
Deferments:
Temporary deferments may be acceptable for the following
conditions:
- Documented

ACCEPTABLE
FORMS OF DOCUMENTATION (of proof of
immunity)
The following
documents are also acceptable proof
of immunity
provided that the dates are acceptable
and the documents are signed and
stamped by the Health Care Provider:
CAN'T
FIND YOUR IMMUNIZATION DOCUMENTS?
If
you are certain that you have received
two doses of Measles vaccine and
one dose of Rubella vaccine in
the past but cannot obtain written
documentation
of the actual dates, it is recommended
that you either have a blood antibody
titer test performed to determine
if indeed you are immune to both
viral diseases.
If
you must register and cannot wait
for the test results,
you can safely receive an MMR
vaccine prior to the registration
process
and a second dose after 28 days
-- assuming you have no medical
contraindications
to receiving the vaccine(s) as
determined by your physician.

WHEN
AND WHERE TO SEND IN YOUR IMMUNIZATION
DOCUMENTS:
WHEN: Before
you plan to register for classes
at FIU for the first time, preferably
at least four(4) weeks before your
register. We recommed fulfilling
this requirement as soon as possible
to minimize delays when registering.
The last month of registration
before each Semester are very busy
times for the Immunization Department.

TRANSFER
STUDENTS
If
you are transferring to FIU you
can submit your immunization information:
- Via fax
to our University Park Campus

our Biscayne Bay Campus at (305)919-5312.

Faxes received will be processed
during our regular
business hours.
Any documents received after
4:45pm will be processed the

needed.

If you
are transferring from FIU,
you can contact
our Immunization Department in order
to release your immunization records
to the transferred institution. Records
can be sent via fax if requested,
or picked up by the student.

ONLINE STUDENTS
Even
if you are not physically coming
to one of our campuses, it is mandatory
to comply with the immunization
process either by submitting proper
records or by providing a letter
from the
department for the class you wish
to take stating that you will be
a fully online student. The letter
must be writtten on the department's
letterhead and must include your
Panther ID number and the signature
of a person in charge. Note that
this letter will only be valid
for one semester, so if your program
requires a long-term online registration,
ensure this is stated in the letter.
HYBRID STUDENTS
(ONLINE AND ON-CAMPUS)
All students who will
be physically coming
to one of our campuses, even if it
is for a short period of time, must
comply with the Immunization requirements.
OFF-CAMPUS
STUDENTS (DISTANT LEARNING, MASTER'S
PROGRAMS) If
you are part of a program where classes
will be taking place at another location
other than one of our campuses, you
have the option of waiving the immunization
requirements by providing us with
a letter from your program.
This
letter must be written on your program's
departmental letterhead and must include
your Panther ID number, the program
you are a part of and the length of the
program.
