THE ROLE OF NUTRITION IN CHRONIC DISEASE
CARE
To help health care professionals meet
the challenges of providing specialized care to the Medicare population,
the NSI developed The Role of Nutrition in Chronic Disese Care.
The manual presents treatment strategies health plans can tailor
to enhance the provision of chronic care. The topics covered include
cancer, chronic obstructive pulmonary disease, coronary heart disease,
dementia, diabetes, failure to thrive, osteoporosis, and hypertension.
The Full Manual has been approved for ten hours of continuing professional
education by the American Academy of Family Physicians, the Commission
for Case Manager Certification and the American Dietetic Association.*

sponsored by the Initiative in partnership with the Washington Business
Group on Health and held in September 1995. The report outlines

A new tool for front line care providers identifying warning signs

unintended weight loss, dehydration, pressure ulcers and tube feeding
complications. These tools, developed by an advisory board including
AHCA, NCCHNR, AAHSA, and with the support of the Senate Aging Comittee
and HFCA are and excellent guide for nusing assistants and the rest
of the care team.
