To Order: Only one item is loaned at a time. All materials are loaned
on a first come-first served basis. If material is out, your name will
be placed on the waiting list. If the waiting list is longer than three
months, you will be notified. Write your name, mailing address, home and
work phone numbers, e-mail address, and the item you are requesting on
a piece of paper. Including a deposit check for $30 made payable to ADA-DPG
11. The deposit will be returned if material is returned in good condi-tion
within 21 days. Items must be insured, as instructed, and returned by
