The regulation was issued on November 20, 2007 and FIU’s response deadline is January 20, 2008. In order to meet this deadline, the Department of Environmental Health & Safety, with the full support of the University Compliance Office, Academic Affairs and OSRA requests your cooperation as follows:

Please do the following in sequence:
- In accordance with existing requirements of the Chemical Hygiene Standard (29 CFR 1910.1450), and the Employee Right to Know (29 CFR 1910.1200), update the inventory of chemicals in use or in storage for your lab(s) using FORM 1
Note: FORM 1, referenced above, is provided for convenience and is the preferred format; however, you may submit your inventory with the minimum required information (chemical name, CAS Number, quantity, and unit of measure) in a format that is convenient for you. If necessary, EH&S will follow-up with you for additional required information.
- Using the information on FORM 1 or your inventory, cross-reference FORM 1 against FORM 2 (DHS Chemicals of Interest) and input in the appropriate cell, the quantity and unit of measure for those chemicals that appear on FORM 2.
- Assure your contact and lab location information is properly filled in on both FORM 1 and FORM 2
- IMPORTANT! Please name your files as follows:
- Chemical inventory or FORM 1 should be named: “FORM1-deptname-bldg-room#” e.g. FORM1-EHS-CSC-162
- FORM 2 (DHS Chemicals of Interest) should be named: “FORM2-deptname-bldg-room#” e.g. FORM2-EHS-CSC-162
- Submit FORM 1 and FORM 2 to ehslabs@fiu.edu on, or before December 21, 2007
