These forms are revised frequently. Please visit the IRB website prior to submitting any protocols to verify that you are using the latest revised form approved by the IRB. Any new information regarding protocols must be submitted on the latest form released by the IRB.

The first page of each sample form provides you with the required elements that must be included in each consent document. Remember that your research population and proposed research may require special considerations. Use the samples as a guide to the preparation of your documents.

Adult Informed Consent

For the acquisition of consent to participate in research approved through Expedited or Full Board Review.

Parental Informed Consent

For the acquisition of consent for a child's participation - Parental consent is required in addition to a child's assent for research approved through Expedited or Full Board Review. Three (3) examples of parental consent are included 1) with parental participation, 2) without parental participation, 3) combined consent/assent form.

Child Informed Assent

For the acquisition of a child's assent to participate - A child's assent is required with research approved through Expedited or Full Board Review.

Verbal Consent

For the acquisition of verbal consent where a waiver of written consent has been requested (or for proposals that meet the criteria for Exempt review).

Informational Letter

For the acquisition of consent by the nature of participation for Exempt approved research. For use with adult populations.
