All Students:
Please read
these important
notes before
following the steps
outlined below:
1. The online
version of the FACT
"Transient Form" is
available
only
to undergraduates.
2. The FIU
Forgiveness Policy
cannot be used for
course(s) taken at
another institution.
3. Transfer credits do
not impact FIU
cumulative GPA.
4.
Course
transferability is
subject to department
and/or university
requirements.
5. Course(s)
not approved by your
academic advisor may
not be transferred.
6. The
"Transient Form" is
only for the term
specified.

Students On
Financial Aid:
If you wish to have
your transient
credits considered
in your Financial
Aid award, please
submit the following
documents to FIU
Financial Aid Office
five (5) working
days prior to last
day to pay fees:
Concurrent
Enrollment Form,
printed copy of the
FACT "Transient
Form, a copy of your
class schedule, and
paid receipt from
(HOST)

University CORE
Curriculum (UCC) Students:
1. Classes taken
to complete UCC
requirements must be
approved by
Undergraduate
Studies Academic
Advising Center and will be
evaluated on an
individual basis. Students must
include the approval
information under
the Student Comments
section.

International
Students with F-1 or
J-1 Status:
1. Refer to the
specific
requirements
outlined on your ISSS Concurrent
Enrollment Form. Failure to adhere
may
JEOPARDIZE YOUR
IMMIGRATION STATUS.

Obtaining a
Transient Student
Application Form:
The Transient
Student Application Form may be printed from FACTS.org at http://facts.org.
To obtain the form from FACTS.org, click
on the link and follow these steps:
1. From the menu, select
Transfer Services
->Transient Student Form.
2. Please read the information provided
on this page before continuing.
Check that the receiving (HOST) is
listed in the Transient Institutions
listing at the bottom
of the web page.
If the
institution is not
listed you will need to submit a paper version of the form.
If the institution is listed click
Continue.
3.
Select your home
institution from the
drop-down box and
enter your
student
ID and PIN,
then click Continue.
Your FACTS PIN can
be
obtained by login
into PantherSoft and
following the link
Academics ->
Personal
Identification
Number (FACTS-PIN).
4. Click on
New Form,
please read the Student Information
Page, then
Next.
5. Finally, choose the
HOST institution
from the drop-down box labeled Select a
School or College. Choose the degree
program in which you
are admitted and
click Next.
Note:
If
you have less than
30 earned college
credits , and/or
have not declared
your major with less
than 60 credits ,
select "Undergraduate
Studies Academic
Advising Center"
instead of choosing
a College /School
program.

How to Complete the
Form :
To complete the form
obtained from
FACTS.org please
follow these steps:
1. Provide all
requested personal
information such as
name, SSN &
address.
Please provide a
valid e-mail since this is how
you will be notified as to whether or not
your application
have been approved
or denied.
2. Under the section
entitled Schools,
choosing the
Receiving (HOST)
Institution.
Remember
if the
institution is not
listed you will need
to
submit a paper
version of the form.
3. Under the section
entitled Term,
select the specific
term and year from
the drop-down menu
provided.
4.
Under the section
entitled Course(s)
to be taken at
Receiving (HOST)
school, please
provide the prefix,
course number, and
credit hours for
each class you will
be taking. For the
sub-section Use of
course, chose the
appropriate category
( General Education,
Major Requirements,
or Electives).
5.
Under the section
entitled Student
Comments, indicate the reason(s)
for taking the course(s) at the HOST
institution rather
that your HOME
institution. Additionally,
if course are being
requested to fulfill
UCC/CORE
requirements you
must first secure
approval from Undergraduate
Studies and enter
the approval
information received
in the
Student Comments box.

Receiving
Admissions to the
Receiving (HOST)
Institution:
1. You
must apply for
admission and pay an application fee at most Florida
Public Community Colleges prior to registering.
Course (s) must be paid for at the time of registration,
even if you wish to have your transient course (s)
considered in your financial aid award. -
Important
Financial Aid Information: If receiving financial aid:
You may be eligible for an adjustment
in your financial aid award. 1. Submit the required documents to FIU Financial Aid
Office five (5) working days prior
to the
last day to pay fees for the term you are requesting
concurrent enrollment. (NO exception will be made to this
deadline). The required documents are: Concurrent
Enrollment Form, printed copy of Transient Student Form,
copy of your class schedule, and paid receipt from the
receiving (HOST) institution.

Transcript
Request:
It is your responsibility to request that an official
transcript be sent to FIU at the end of the semester from the
school that you attended.

Students
who have finished
taking courses
at FIU must request
a transcript to
be sent to their
home institution. Please
use the following
