[a] The Non-Degree Seeking Student Application Form:
If you have never attended or have not registered for the past four semesters, you must complete and sign a Non-Degree Seeking Student Application form located at http://www.fiu.edu/orgs/broward/pdf/formnondegree.pdf. This form may only be submitted in-person or via postal mail. Students planning on attending courses in Broward may drop off the form at the Broward ~Pines Center in Room 101. Students who prefer to send the form via postal mail should use the following address: FIU Broward - Pines Center 17195 Sheridan Street, Pembroke Pines, FL 33331

[b] How to Register:
Once, we receive your completed Non-Degree Form, your unique identification number, commonly referred to as your PantherID, will be available online within two (2) working days. To obtain your PantherID please visit http://sis.fiu.edu , click on the link - PantherID Look UP and provide the information requested. To create your password, click on the link - Login to PantherSoft on the http://sis.fiu.edu   web page, enter your PantherID and use your birth date - [MMDDYYYY], as the default password. You will be prompted to change this password and to create a unique one which we advice that you keep secure.

[c] Health / Immunization Requirements:
As a prerequisite to registration, Florida International University requires all students born after December 31,1956 to present documented proof of immunity to measles (Rubeola) and German measles (Rubella). Documentation may be submitted directly to the Broward ~Pines Center in Room 101 or University Health Care and Wellness Center, University Park; or HM 110, Biscayne Bay Campus.
