To receive your plate:

• Download the plate order form.
• Complete sections 2 and 3 of the form.
• Sign Section 3 on “Owner/Applicant’s Signature” line
• Attach a photocopy of your Florida Driver’s License (front and back)
• Mail or fax both documents to:
