6.10 FINANCIAL AID

Effective: June 29, 1994

PURPOSE
To state the availability of financial aid.

AUTHORITY/SOURCE:
Provost and Vice President for Academic Affairs
Florida Administrative Code, 6C8-3.018

POLICY
The University provides various types of financial aid to students. Information can be obtained from the Office of Financial Aid and the Dean of Undergraduate Studies, as well as from the academic departments, for opportunities of a more specialized nature.  Information on minority student assistance programs can be obtained from the Office of Minority Student Services.

Award criteria: Those students awarded financial assistance must meet criteria established by the sponsoring agency.

Financial Aid Administration: The Financial Aid Office is responsible for authorizing disbursement of financial aid according to the terms and conditions specified by the authorizing agency.

PROCEDURE
The student applies for financial aid by obtaining application materials from the Office of Financial Aid and providing the requested information. Once a need assessment has been completed by the office, a notice of award will be mailed to the applicant. The student must accept the award within 15 days or request an extension if there is any problem. If the student fails to do this, the award will be canceled. All financial aid recipients must go to the Cashier's Office to have their class schedule validated or pay required fees prior to the date shown on the Fee Due Notice received during registration. Failure to do so will result in the cancellation of the student's registration for the semester.

6.20 MINORITY STUDENT SERVICES

Effective: June 29, 1994

PURPOSE
To notify minority students of the availability of services.

POLICY
Minority students can receive counseling and academic tutoring provided by the Office of Minority Student Services. Information can be obtained from that office.

PROCEDURE
Students contact the office and arrange assistance.

6.30 OFFICE OF DISABILITY SERVICES

Effective: June 29, 1994

PURPOSE
To notify disabled students of availability of services.

Director of Disabled Student Services

POLICY
Disabled students can receive assistance in various aspects of their education from the Office of Disabled Student Services. This office provides notetakers, readers, sign-language interpreters, library and lab assistance, registration assistance, and limited academic counseling and other related services. The office communicates with professors about the particular needs of disabled students.

PROCEDURE
Students contact the office to discuss their needs.

6.40 INTERNATIONAL STUDENT SERVICES

Effective: June 29, 1994

PURPOSE
To notify international students of the availability of services.

Director of International Student Services.

POLICY
International students may receive assistance with visas, immigration status, limited academic advising, matters of employment, and other related services from the Office of International Student Services, and other related services.

PROCEDURES
Students contact the office to receive assistance.

6.50 LIBRARY

Effective: June 29, 1994

PURPOSE
To clarify privileges of students using the University Libraries.

AUTHORITY/SOURCE:
Provost and Vice President for Academic Affairs
Florida Administrative Code, 6C-5.007

POLICY
Borrowing privileges at the University Libraries are extended to students, faculty, staff, alumni and Library Associates.

Intercampus Loan
Intercampus Loan service provides access to the collections of the libraries regardless of campus location. The service is available to Florida International University faculty, staff and currently enrolled students.

Interlibrary Loan
For materials not owned at the libraries, interlibrary loan is available to Florida International University faculty, staff and currently enrolled students. Most interlibrary loan materials can be obtained without a charge. However, some charges may apply. These charges will be the responsibility of the requester.

Failure to pay fines or for lost books may result in:

1) Denial of further circulation privileges
2) Denial of permission to register for classes
3) Withholding of transcripts
4) Withholding of diplomas

PROCEDURES
Students may obtain library cards by presenting a current student photo I.D. card and filling out an application form at the Circulation Desk. Lost or stolen cards should be reported to the Circulation Desk immediately.

Patrons are responsible for all materials borrowed on their library card and for returning or renewing materials on time. Fines will be assessed for overdue, lost, and damaged materials. A fine schedule is available at the circulation desk.

Excessive abuses may result in denial of circulation privileges.

All overdue fines and charges for replacement of materials must be paid at the University Cashier's Office.

The location and circulation status of SUS materials may be checked at any LUIS terminal. Information on FIU materials and intercampus loan are available at the Circulation Desk. Information on interlibrary loan is available at the Interlibrary Loan office.

If the student drops enrollment at the University, the card becomes invalid and borrowing privileges are suspended for that semester. The library card may be revalidated if the student registers in the future.

6.60 STUDENT HEALTH SERVICES

Effective: June 29, 1994

PURPOSE
To notify registered students about health care

AUTHORITY/SOURCE:
Provost and Vice President for Academic Affairs
Vice President for Student Affairs

POLICY
To be eligible to use the SHS, students must be fee-paying and registered at the University (including the student health fee). Exceptions may be made for exchange programs. The clinic provides consultation and referrals for day-to-day health concerns, including gynecological services. If lab tests are ordered by the care provider, they are performed for a nominal fee. There is a small pharmacy on the premises for filling most common prescriptions.

At pre-registration time, immunization services are also provided for a fee.

Additional Student health Insurance is available for a reasonable price from an approved insurance provider. Brochures are available at the office.

PROCEDURE
Students call the Student Health and Wellness Center to make an appointment.

6.70 STUDENTS RIGHTS AND RESPONSIBILITIES

Effective: June 29, 1994

PURPOSE
To make students aware of their rights and responsibilities.

Provost and Vice President for Academic Affairs

POLICY
The University has developed policies and procedures on the rights and responsibilities of students and a code of conduct assuring that these rights can be freely exercised without interference or infringement by others.

If departments have policies and procedures that go beyond the basic requirements, their policies and procedures manuals must include relevant explanations and must be made available to the students.

PROCEDURE
The code of conduct, academic misconduct policies, student grievance procedures and policies on student records are reported in detail in the Student Handbook.
